Attention CNA, CCT, EKG and Phlebotomy students – Please be sure to read
completely the course descriptions and requirements.
Students must bring the following items to the first class:
Immunizations:
Required for CNA, Cardiac Care, EKG and Phlebotomy
classes.

Completed
immunization form.
Immunization records may be attached to this form. If vaccination records
are not available then titer levels stating immunity may be used in lieu of
having the immunizations re-administered.

2-Step Tb test – please see date range specifics for each course.
Background Checks:
Required for CNA, Cardiac Care and EKG classes.
·
SLED background check done within 30 days of start of class. Clean record
required for 10 years.
Textbooks:
Required first night of class.
Uniform policy:
All CNA, EKG, Cardiac Care Tech, and Phlebotomy must wear white
scrubs and white closed-toe shoes to all classes and clinicals.
